Please read before submitting. Before setting up your tag, please create a profile first here. You will require this when you need to revise your tag.

 

**For new tags only. To revise/update FOBs/Tags, please use the revision page (click here). If you own multiple tags, a profile needs to be set-up for each. You cannot update multiple tags with a single profile.

Please fill in ALL fields. If you do not have information for a field, simply fill in N/A.

 

To make revisions, login to the account used to set up the tag. Fields should auto-populate with your previous form information. Filling in text/uploading files will overwrite the previous submission. Deleting auto populated text will overwrite the existing information. Once overwritten, the previous submission will not be available. Please ensure you are only updating the fields that need to be updated.
